There seems to be a problem with your website, I would like to make a right to choose appointment however I get the error which is in the image below, I have tried 3 different browsers, my phone and my laptop and still have the same error.
My go rejected my right to choose referral and I would like to self refer or find out what I could do next to help my situation as I am struggling mentally really badly.
