RTC titration after RTC assesment

Hi, has anyone else experienced an issue when trying to swap RTC service provider for titration? Our assessor (medinet/HealthHarmonie Minds) doesn’t yet offer titration for kids but they told me I could use RTC to find someone else to do this. However several clinics (Montrose, ADHD360, CARE) have told me I can’t. They said RTC can only be used once and if the assessor doesn’t offer titration then tough luck AND they won’t accept report done by someone else, even if it is an NHS approved RTC assessor. Looking at the rules this doesn’t seem right:

NHS England’s 2026/27 NHS Payment Scheme: Attention-Deficit/Hyperactivity Disorder (ADHD) & Autism Diagnostic Assessment Services Payment Guidance, section 2, page 4, directly addresses this situation.

Requirement 2 provides that where a service cannot deliver every part of the care pathway, it should have clear arrangements enabling patients to move smoothly into another service, with a timely and coordinated handover. The guidance specifically recognises that patients should not be assessed or titrated in isolation from the wider pathway and that failures in coordination contribute to delay and additional system pressure.

Requirement 4 states that unnecessary reassessment should be avoided and that ADHD diagnostic reports from independent providers, including providers eligible to deliver services under Right to Choose, should be accepted by NHS-commissioned titration services unless there is a clear clinical reason not to.

Clinical Partners WILL accept someone else’s report but their RTC service has a 10 month wait. Has anyone else got experience/advice on this situation? Thanks.

Hi We are in a similar situation - we were referred to Psychiatry UK via RTC but having been told it would be 10 month wait for titration, and now 12 months, we tried to switch to Medinet because they said they were taking adult titration with a diagnosis from another provider. However we have been blocked by our ICB. They have put new criteria in place that mean we can only get services from providers who offer a face to face appointment within 60 minute drive from our home, which they are not. Additionally, GP said that if we move away from the original RTC route then they would not be able to offer shared care.
